In connection with, and as a condition of, the undersigned’s attendance at events conducted by True Focus Coaching, Inc. and their affiliates (“Promoter”). I confirm, acknowledge and agree as follows:

- The Event is purely an educational Event put on by Promoter.
The education and information given at the Event is intended for a general audience and does not purport to be, nor should it be construed as, specific advice, therapy or counseling tailored to any individual.
All materials, procedures, policies, and standards, all teaching manuals, all teaching aids, all supplements and the like that have been or will be made available to me or any other participant in this program, from Promoter, True Focus Coaching, Inc. or her designated facilitators, or any other source, oral or written, are for my personal use in or in conjunction with this training program only.
Further, I recognize and agree that some or all of the aforementioned material or information may contain professional secrets and/or private training methods and hence will be and will remain the sole property of True Focus Coaching, Inc.
I understand and agree that the information from this program is for my personal use only, and may not be sold, tape recorded, videotaped, shared, taught, given away, or otherwise divulged without the express written consent of Promoter or its designated agent. I also understand that I, while free to share my experiences and my personal results of this training, will keep the experiences and statements, oral or written, of all other participants in this training in the strictest of confidence.
Release & Liability Waiver
In consideration of and as part of my payment for the right to participate in Promoter’s Event, the undersigned, my heirs, executors, administrators, successors and assigns do hereby RELEASE, WAIVE, ACQUIT, DISCHARGE, INDEMNIFY, DEFEND, HOLD HARMLESS AND FOREVER DISCHARGE True Focus Coaching, Inc., and its owners, subsidiaries, principals, directors, employees, agents, heirs, executors, administrators, successors, and assigns and any of the training instructors, guides, staff or students taking part in the training in any way as well as the venue where the Events are being held and any of its owners, executives, agents, or staff OF AND FROM ALL ACTIONS, CAUSES OF ACTION, CONTRACTS, CLAIMS, SUITS, COSTS, DEMANDS AND DAMAGES OF WHATEVER NATURE OR KIND IN LAW OR IN EQUITY arising from my participation in the Event.
I do hereby acknowledge that the activities at the Event may pose known and unanticipated substantial risks of physical injury, psychological or mental injury and/or death, disease or illness and/or damage to or loss of personal property. I hereby knowingly and voluntarily assume any and all risks and do agree and acknowledge that my participation in the Events and any of the events or programs associated with the Event is of MY OWN FREE WILL AND COMPLETELY VOLUNTARY, regardless of anything stated or implied.
I agree not to teach others any of the activities conducted at the Event unless I am fully trained and qualified and earn my living doing so.
I am fully responsible for any and all decisions I make in conjunction with the Event and I alone will determine which activities I will participate in. I further understand that I DO NOT HAVE TO PARTICIPATE IN ANY ACTIVITY AND I MAY WITHDRAW MYSELF FROM PARTICIPATION AT ANY TIME. I agree I will use my own free will and discretion, considering my physical and mental health, when deciding if it is appropriate for me to participate in any activity.
- I also acknowledge that (i) I have read this document, (ii) I will inspect the facilities, grounds and equipment prior to being involved in any activity, (iii) I will not participate until I have conducted such inspection and (iv) my participation is my acceptance of the facility, grounds and equipment (hereinafter (“Sites) as being safe and reasonably suited for the purposes intended and (v) I voluntarily sign this document.
I indicate by my signature below that I have had sufficient opportunity to read this entire release and waiver document and acknowledge that I have understood it, and agree to be bound by its terms. No representations, statements or inducements, oral or written, apart from the foregoing written statement, have been made.
PHOTOGRAPH / TESTIMONIAL CONSENT AND RELEASE
I, the undersigned, of legal age, hereby grant, irrevocably and perpetually to TRUE FOCUS COACHING, INC., (TFCI) and their subsidiaries, affiliates, successors and assigns thereof, the following:
- The right to photograph, video and record my name, voice, appearance, likeness, and/or written testimony along with any material furnished by me, in whole or in part, in any program provided by TFCI, or any affiliate, subsidiary or assign of TFCI.
- The right to publish, exhibit and distribute the use of my name, voice, appearance, testimonial and/or likeness along with any material furnished by me, in whole or in part, worldwide, for any commercial purpose, including but not limited to the advertising or solicitation of business, by any means of mass and/or electronic media, including but not limited to print, radio, television and promotional materials, events and/or marketing plans.
I hereby release and agree to indemnify and hold harmless TFCI and their subsidiaries, affiliates, successors, assigns, officers, directors, agents, owners, employees, thereof, from any and all claims, actions, causes of action, damages, expenses, court costs, attorney fees, liability damage or judgment brought forth as a result of my participation or testimonial materials furnished by me.
I hereby acknowledge that I have not been paid or otherwise compensated, nor will I be compensated, for my participation to record and publish my name, voice appearance, likeness, and/or written testimony, along with any material furnished by me, in whole or in part. I further certify that all material, whether verbal, written or exhibited by me has not been scripted and represents my individual opinions and beliefs, and are true and correct to the best of my knowledge.
